What Preventive Care Looks Like in Your Early 40s

When I first wrote about turning 41, I treated preventive care a little too much like a checklist. The better way to think about your 40s is as a time to review what is due now, what is coming next, and what changes because of your own history.
Breast cancer screening
For average-risk women, the U.S. Preventive Services Task Force recommends screening mammography every two years from ages 40 through 74. Higher-risk patients may need a different plan.
Cervical cancer screening
Cervical screening depends on age, prior results, HPV testing strategy, whether the cervix is present, and any history of significant cervical precancer or cancer. It is not automatically an annual Pap test.
Colorectal screening is coming soon
For average-risk adults, colorectal cancer screening begins at age 45. That can involve stool-based testing, colonoscopy, or other accepted strategies depending on the patient and screening plan.
Vaccines are not one-size-fits-all
Annual influenza vaccination remains routine. Tdap/Td timing, hepatitis B vaccination, COVID vaccination, and other vaccines depend on age, prior doses, health conditions, pregnancy, and risk. Shingrix is routinely recommended starting at age 50 for immunocompetent adults, and earlier for adults with certain immunocompromising conditions.
The cardiovascular basics still matter
Blood pressure, cholesterol, diabetes risk, nicotine exposure, sleep, activity, family history, and weight trajectory often matter more than adding a long list of low-value tests simply because someone had a birthday.
What I would tell my 41-year-old self now
Preventive care is not about collecting tests. It is about using age, history, family risk, and prior results to decide what actually improves the chance of staying healthy.
