CagriSema REDEFINE 1: What the Published Phase 3 Data Show

When headline trial results come out, I always want to see what happens when the full study is published. That is where the details start to matter.

The Phase 3 REDEFINE 1 trial of CagriSema has now been published in the New England Journal of Medicine, giving us a much fuller look at the combination of cagrilintide and semaglutide.

Two ways of looking at the weight-loss result

REDEFINE 1 included 3,417 adults with obesity or overweight and at least one related complication, without diabetes. At 68 weeks, average weight reduction with CagriSema was 20.4% in the treatment-policy analysis, compared with 3.0% with placebo.

A separate analysis estimating the effect if participants stayed on treatment showed 22.7% average weight reduction with CagriSema, compared with 16.1% for semaglutide, 11.8% for cagrilintide, and 2.3% for placebo.

Those two numbers—20.4% and 22.7%—are not competing claims. They answer slightly different questions about how trial results are analyzed. I think that distinction is worth understanding whenever dramatic percentages make headlines.

The combination still looks like more than either component alone

The study is especially useful because it included the individual components. That lets us see the added effect of combining GLP-1 receptor activity with an amylin analogue rather than comparing the combination only with placebo.

Gastrointestinal effects were common, as expected with this type of treatment. Importantly, the protocol allowed investigators to adjust dosing based on tolerability, which is one reason I keep coming back to the idea that dose flexibility matters.

Where the FDA fits now

With pivotal Phase 3 results now published, the CagriSema story has moved much closer to the regulatory stage. An FDA application is not built from one exciting number; it includes the full clinical data package along with manufacturing, labeling, safety, and other information.

But this is exactly the kind of large, controlled Phase 3 evidence that becomes central to that application. The next major milestone I will be watching is the formal submission.

Why I keep following this one

CagriSema represents something bigger than one product. It is evidence that the next generation of weight-management treatment may involve deliberately combining complementary hormone pathways rather than continuing to rely on GLP-1 alone.
